didn't tighten the axle nut all the way or didn't tighten the set screw is two possibllities that come to mind. Either that or your bearings are shot.
Need help with axle
I just put in my new durablue 3+5 axle in. Now after i put it in I tightend everything on it and went for a test drive. After I got back i notice the whole axle moved to the left about an inch. I have no idea why its doing this. Any of you guys have any ideas?

i replaced the bearings and tighten the bolts and everything
What axle nut are you using?
the one that comes on the durablue axle i'm guessing there must be a missing part or something
Karnivore nailed it, I would say it would have to be on of those things. Try tightening the axle nut again and resetting the set screw and see if it moves again.
