didn't tighten the axle nut all the way or didn't tighten the set screw is two possibllities that come to mind. Either that or your bearings are shot.
Karnivore nailed it, I would say it would have to be on of those things. Try tightening the axle nut again and resetting the set screw and see if it moves again.
